Online Report Form
The Saint Mary's 校园安全 Department offers an online reporting service for your convenience. Through Online Reporting, you can report non-emergency crimes and submit Anonymous Tips if you have information that might be of interest to 校园安全.
If you are facing an emergency that requires immediate attention, please call 校园安全 at 925-631-4282 or dial 911 before completing this referral form. Note that while this form is reviewed daily by 校园安全 to support the Saint Mary’s community, it is not intended for emergency response.
Campus Security Authority (CSA) Reporting Form
The form below is the CSA Report Form. A Campus Security Authority (CSA) is designated by the College under the Clery Act to receive and report criminal incidents to the Department of 校园安全 for inclusion in the College’s Annual Security Report. CSAs have completed training in handling crime reports, victim support, and related school policies. This form may also be used to assess whether a timely warning or security alert should be issued to the College community. College CSAs should complete this form and email it to psadmin@shuwukeji.com.

保密 is a priority for us at 校园安全. Your report will remain private and anonymous upon request. All emails will be deleted after being forwarded to the Director of 校园安全. Please note that if the email concerns a 第九条 violation or other mandatory-reporting issue, confidentiality cannot be guaranteed.

Service complaints and allegations of misconduct will be investigated promptly and with fairness, 中立, 和公正性. The Executive Director of 校园安全 will review all investigations and, in collaboration with Human 资源, decide on appropriate corrective measures. Those who file formal complaints will receive written notification of the outcome.
